Need part # for upper heater hose

Just like title states, I am looking for a part number or source for the upper heater hose that goes from the firewall to head. Mine is molded and has the burp tee spliced in. I've searched through many threads and have not found anything. eBay and my IAP catalog yields no results either. This is that hose. The curve to go over to the engine is not molded into the hose. I've never seen one that was.
You simply bolt it to the heater tube coming out of the firewall, then bend it over to go to the head.

It has a sharp 90 degree turn as it come off the firewall, and then it is straight.
